As a client support worker at OCMO, I guide people in finding suitable care and support within the social domain. I support clients with questions about the personal budget, known as PGB, help them search for appropriate care, and support people with Alzheimer’s disease and their family members.
I believe it is very important to listen carefully and to look for suitable solutions together, based on someone’s personal circumstances. Confidentiality and personal contact are central to this. Because I speak both Dutch and Turkish, many clients feel at ease with me more quickly. My goal is to strengthen clients and the people around them, so they can shape their own lives as much as possible.
With an eye for the bigger picture, I support people who ask for help without judgment. I focus on contact and connection, and from there I help work towards suitable solutions. My specializations are mainly in finance, processes and procedures. That is why I particularly enjoy working in the field of WMO, such as home adaptations and assistive devices, as well as WLZ-related matters such as PGB, calculation modules, budget plans, and more.
Alongside my work as an independent client support worker, I also work as an energetic coach. This enables me to create a strong connection between the practical side and the human side. The groups I often work with include people with ZEVMB, EMB and autism. I am happy to think along and help with complex situations.

In my role, I support clients with a mental health care indication. My choice to work with this target group comes from my extensive experience and commitment. My language skills include Dutch, English and Surinamese.
My name is Denise Swam. Since 1 November 2024, I have been using my experience, knowledge and skills as an independent client support worker for OCMO in the Rotterdam and South Holland Islands service area. Choosing OCMO was a conscious decision, because the organisation focuses on people with a migration background.
